What is a Home Care Package?

Yellow Door Care is all about keeping people where they want to be: at home. In order to make this possible without stress to family and friends, the Home Care Package was created as a government initiative. It successfully addresses all the factors which make staying at home an option where otherwise a Nursing Home or longer stay in hospital might have been the only choice.

Choice
One of the most empowering aspects of the Home Care Package is that it does allow your loved one the choice of remaining in their own familiar and comfortable environment. It is well known that changing their environment in some people can lead to distress, disorientation and unhappiness. If that can be avoided safely, it is an objective that Yellow Door Care views highly in the care of their clients, and why they actively pursued the intensive process of becoming a Registered Provider.

How much does it cost?
Home Care Packages provide ‘myagedcare’ government subsidised services. It is available to people usually aged 65+ or 50+ for Aboriginal and Torres Strait Islanders. The funds may be partly or fully funded by the Australian Government.The funds can be allocated according to personal choice, providing help which ranges from meal preparation and shopping to all aspects of personal hygiene, medications and maintaining the home environment. The package is tailored to needs as they arise, so that once in place it can be increased to a higher level of help, ultimately prolonging the time able to remain in their own home.

What if you are ineligible for government funding?
If you are ineligible or waiting for funded services to become available, you can still access privately funded services from Yellow Door Care.

A home visit can be arranged which provides reassurance to the clients who may find the idea of someone coming into their home confronting or intrusive. They are soon put at ease when they realise that the care is sensitive, respectful and comforting. It relieves any burden or worry that may already be on loved ones, and this often provides a sense of relief to everyone involved.

Optimising safety
The concern for families is often whether or not their elderly relatives are safe, especially those who live alone. When a Home Care Package is in place, it means someone is coming regularly into contact with them. Any signs of decline or inability to cope is picked up quickly. Illness can be acted upon, and Yellow Door Care also provides for clients to be taken to appointments. This involvement also spills over into issues with the home, so that services can be arranged to fix that blocked toilet, broken washing machine or light bulb that needs changing. Pets can be taken for a walk and gardens can be watered. Peace of mind comes from addressing all issues and timely communication is maintained with family members.

Other Services
Yellow Door Care staff will assess the home for additional safety requirements like hand rails, raised toilet seat, ramps or a call direct button from the ambulance service.
Other services can be organised like physiotherapy, social activities, therapeutic massage, appointments with opticians or other medical specialists.
Importantly nursing care is provided by qualified professional staff following a visit to hospital. Some of these services can be short term and added as required to the overall package.
