What Is Restorative Care?

The Restorative Care Pathway offers up to 16 weeks of focused services designed to help you improve or maintain your physical ability, mobility, and wellbeing.

It is completely goal-based, meaning your support is shaped around what you want to achieve — whether that’s walking safely around your home, rebuilding strength, returning to the community, or managing daily tasks more independently.

All Assistive Technology and Home Modifications provided through our service must be delivered as part of our wrap-around services and support model. We do not offer stand‑alone equipment or home modification orders without proper assessment. All AT and Home Modifications must be prescribed and coordinated by one of our Associated Providers to ensure they are safe, appropriate, and aligned with your overall support plan.

How to Get Started

  1. Book a Care Consultation — talk with one of our Care Partners about your needs.
  2. Complete Your Aged Care Assessment — we’ll support you through it and help you articulate your needs for equipment or modifications.
  3. Plan & Prescribe — we’ll work with you and Allied Health professionals to decide what assistive technology or modifications are right.
  4. Implement & Adapt — once approved, we’ll manage sourcing, installation, training, and maintenance.

Yes. Eligibility is determined through a My Aged Care assessment, where your potential to improve with short-term support is evaluated.

Yes. If ongoing support is needed, we’ll help you transition into regular Support at Home services or update your support plan through a reassessment.

No. Restorative Care is additional to your quarterly Support at Home budget, so you won’t use your main allocation during this short-term program.
