What Is the End-of-Life Pathway?

This pathway provides intensive support for up to 12 weeks (extendable to 16 weeks if needed) to help you remain comfortably at home. It includes coordinated clinical care, personal support, emotional support, and help for family and carers.

It is available through the Support at Home program and is fully centred around dignity, comfort, and respect.

What You Can Use Your Support at Home Budget For

The End-of-Life Pathway brings together all the care you need to stay comfortable at home. This includes gentle clinical support like nursing for symptoms, help with medications, pain management, and regular check-ins, all coordinated with your GP or palliative care team. You’ll also receive personal support with bathing, grooming, moving safely, positioning, eating and staying hydrated.

Families are supported too, with guidance, emotional support, practical help, and respite so they can take breaks when needed. To make your home safer and more comfortable, equipment such as hospital beds, cushions or mobility aids can be arranged, along with assistive technology or small home modifications if appropriate. You’ll also have access to companionship and emotional or spiritual support, with respect for your cultural or personal wishes.

Who Is Eligible?

The pathway is available to anyone who is clinically assessed as being in the final months of life.
Your My Aged Care assessor will determine eligibility during your assessment.

How to Get Started

If you or your family member may be eligible, contact us for a gentle, supportive conversation. We can guide you through assessments, planning and setting up the pathway quickly and respectfully.

  1. Book a Care Consultation — talk with one of our Care Partners about your needs.
  2. Complete Your Aged Care Assessment — we’ll support you through it and help you articulate your needs for equipment or modifications.
  3. Plan & Prescribe — we’ll work with you and Allied Health professionals to decide what assistive technology or modifications are right.
  4. Implement & Adapt — once approved, we’ll manage sourcing, installation, training, and maintenance.
