Why This Matters to You
As part of the Support at Home program, the AT-HM scheme gives you separate funding just for equipment and home changes — so you don’t have to use your regular care budget for these essentials.

What Can I Get Through AT-HM?
If you’re eligible, the AT-HM scheme can fund:
- Assistive Technology: Walkers, wheelchairs, toileting supports, shower chairs, and other equipment to help with daily living.
- Home Modifications: Changes to your home environment — like grab rails, ramps, stair-lifts, or redesigning bathroom layouts — to make your space safer and more accessible.

Wrap-Around Services for Assistive Technology and Home Modifications
All Assistive Technology and Home Modifications provided through our service must be delivered as part of our wrap-around services and support model. We do not offer stand‑alone equipment or home modification orders without proper assessment. All AT and Home Modifications must be prescribed and coordinated by one of our Associated Providers to ensure they are safe, appropriate, and aligned with your overall support plan.

How Yellow Door Care Supports You Through AT-HM
The Support at Home program also includes three specialised pathways designed to support you through short-term or significant life changes:

AT-HM Fees, Eligibility and What’s Covered
Your Costs
- AT-HM funding from the government is separate to your regular Support at Home budget, so you don’t need to “save up” from your quarterly care allocation.
- There may be contributions:
- For assistive technology items: you may have a co-contribution, depending on your means assessment.
- For prescription, assessment, wrap-around services (like training to use the equipment): these are considered clinical supports and are fully funded.
Who Is Eligible
- Anyone approved under the Support at Home program can apply for AT-HM funding during their assessment.
- If you transitioned from a Home Care Package, your unspent HCP funds must be used first before entering the AT-HM scheme.
- In some cases (e.g. complex home modifications), your provider may request a plan review to increase your AT-HM funding if the initial tier isn’t enough.
What’s Not Covered
Some items are not eligible under AT-HM:
- Items more suited to other government programs
- Discretionary household expenses that aren’t linked to clinical need
- Workplace equipment
- Items for people under 65 (unless in specific conditions)
How to Get Started
- Book a Care Consultation — talk with one of our Care Partners about your needs.
- Complete Your Aged Care Assessment — we’ll support you through it and help you articulate your needs for equipment or modifications.
- Plan & Prescribe — we’ll work with you and Allied Health professionals to decide what assistive technology or modifications are right.
- Implement & Adapt — once approved, we’ll manage sourcing, installation, training, and maintenance.
